The Calais School Department accepts funding through the Elementary and Secondary Act (ESEA) of 1965. As part of ESEA, the Calais School Department is required to provide an opportunity for public comment on the consolidated application and consider these comments prior to the submission of the application and will be doing this through July 16, 2021. St. Croix Family Health Center staff are preparing for your students’ second dose of Pfizer Covid19 vaccine!
Your students will have the full immune response by June 25 and can enjoy their summer knowing they are protected!! It’s so exciting and will help keep schools safe and open for the 2021-2022 school year.
Students and families are reminded that students must
have their completed health history form and vaccine card
in hand on the 11th. (they all were given the health history form when they received their first vaccine. ) The health history form is the same one they completed for the first dose, If you need another consent form or have any questions, please email the school nurse at email@example.com
The clinic will be at Calais High School gymnasium. Please plan to arrive around 9 AM and wait 15 minutes after the shot is given. The clinic is expected to last no more than 1 hour.
Calais Middle/High School will be hosting a Pfizer Covid Vaccine clinic on Friday, May 21. The second dose will be given at the school on Friday, June 11 (yes, the school year is over, but we will still hold the 2nd dose clinic). Please commit to both dates.
The clinic will be run by St. Croix Regional Family Health Center in collaboration with the Maine CDC. The vaccine is available to students ages 12-19 at no cost. Vaccine Information, registration and consent forms are attached. Parents must return the registration and consent forms no later than May 19. You can print them out, complete them and sign them. Paper forms will also be distributed in homerooms on May 14 and May 17. We will need you to return a paper copy of the forms.
